Paid medical bills being sent to collection agency

I have been receiving a bill from this company since May 2012 for $135.00 which has been paid in full by my insurance company. I have had several conversations with them and have faxed the Explanation of Benefits from my insurance stating that it has been paid. Yet, they continue to send bills. Today I received a collection notice from them. How do I get this to stop? I have proof of payment, but no one seems to be able to return my calls or answer my faxes. I have told them to cease and desist with the billings. Now I have been threatened with a lawsuit. What are my options?
