I received a notice to pay a debt collector after 4 years of not receiving a bill. Do I have to pay this?

If the debt is yours, then you are legally obligated to pay it. The fact that you have not received a bill for the money you owe in 4 years does not change that fact. It's likely that the medical provider with whom you incurred the debt recently went through all of its past due accounts, noticed that your account was very past due and decided to turn it over to a debt collection agency.
